Anaphase, is the stage of mitosis when chromosomes separate. Each chromatid moves to opposite poles of the cell, the opposite ends of the mitotic spindle.

Telophase is a stage in mitosis which reverses the effects of prophase and prometaphase events. During those events, the nucleus was dissolved and the chromatin in the cell was condensed into chromosomes. Telophase thus "cleans up" the secondary after-effects of mitosis.
